Skilled Nursing 

  • Wound care, Dressing changes, Catheter care/drain, IV Therapy, Teaching medications and disease processes. Teaching diet and nutrition, Pre and Post -Operation teaching. Establish a safe home plan of care. Tube feedings, evaluation and assessment of unstable physical symptoms, Labs along with other skilled care.

Podiatry

  • Dr. Jerry E Zayid is  our Podiatric Medical doctor which medically and surgically manages care of the lower extremity. The podiatric physician is a health professional who is involved with examination, prevention, diagnosis, and treatment of foot disorders by physical, medical, and surgical means. He is trained to detect the signs of general bodily disorders which may appear first in the lower extremity, such as diabetes or circulatory disorders. When such diagnoses are made, the podiatrist consults with the patient's family doctor concerning continuing treatment

 

 

Physical Therapy 

  • Limited mobility/gait disturbances, Limited ability to transfer, Loss of range of motion, Loss of strength/muscle tone, Musculoskeletal pain, Establish home program, Massage, Prosthetic training, Home safety, DME equipment training.

Speech Therapy 

  • Hearing loss, Dysphasia, Dementia/Cognitive problems, Memory Loss, Aspiration pneumonia, Language disorders, Aural rehabilitation, Non-Verbal Communication

Home Health Aid

  • Assist with personal care (must do hands on care) Stays with patient only until the care of plan from nurse is completed.

Occupational Therapy

  • Patients with CVA diagnosis, Energy conversation techniques, COPD/lung or heart conditions, Upper body weakness or trauma, Exacerbation of arthritis, Home safety/transfer training, ADL training, Cognitive training, Fine motor coordination, DME equipment training, Establish home program, Perceptual motor training.

Medical Social Worker

  • Assess social/emotional/economic and environment, Counseling for long-range planning and decisions, Adaptation to disease limitation, Community resource planning, short-term therapy, Crisis interventions/family problems, Suspected abuse or neglect, Non-compliance issues.
